I declare that the information on this enrolment form is to the best of my knowledge, true, accurate and absolute at the time of this enrolment.

I acknowledge that any false information and not disclosing relevant information for enrolment of this qualification will result in the cancellation of my enrolment at FAC.

I have read, understood and am voluntarily agreeing to comply with all stated within the FAC Diploma Student Handbook. I understand that it is my full responsibility to provide all relevant and required documentation and answer all questions truthfully.

I also acknowledge that I am responsible for keeping a copy of all signed correspondence (including, but not limited to, this Enrolment Form and the Letter of Offer) and receipts of any payments of tuition fees (scheduled and non-scheduled).

I am aware that if any information provided within this enrolment changes, I must notify FAC within seven (7) days of the change, otherwise, my enrolment at FAC may be cancelled at the discretion of FAC.

I have viewed, read, understood and voluntarily agree with the payment Terms and Conditions.

I further understand that the enrolment fee of AUD $250.00 is a NON-REFUNDABLE amount under any circumstances.

Once formally enrolled, I give consent to FAC to use and publish my name, photographs and/or videos in any publications (including but not limited to written and/or multimedia reports, guides, brochures and Institute marketing materials) [the “Material”] for distribution anywhere in the world and on associated websites for educational, promotional or reporting purposes. I am aware that any information published on the Internet is accessible by millions of users from all over the world. Further, the Material will be indexed by search engines and may be copied and/or used by any such user in contravention of the Institute’s intellectual property rights. As such, I will have no control over the Material’s subsequent use and/or dissemination. I acknowledge that:

1. I am not entitled to any remuneration, royalties or any other form of payment arising from the use and/or reproduction by the Institute of the Material; and

2. I have the right to ‘opt out’ shall I wish by advising an Administration Support Officer of my desire to do so via email.
